The undersigned, hereafter called the student, does hereby take temporary possession of the equipment described above from Dayo Scuba, for use during the scuba class mentioned above, subject to the following terms and conditions:

1. Student agrees to pay full value of class items, plus any costs incurred in collection in case of loss, or full repair cost in case of damage.

2. Student agrees to pay $25.00 per day, each day equipment is kept without the Dayo Scuba's consent, for example, the gear is kept past the date it was supposed to be returned.

3. Student agrees to return all equipment well cleaned and free of sand, salt or other contaminants.

4. Student further agrees not to loan or rent this equipment to any person not properly certified in its use.

5. Student will use stated equipment at their own risk and assumes all responsibility for damages to others or to the property of others or to the Student or their property, resulting in any manner from Student's use of the equipment while being used by the Student. Student will hold Dayo Scuba harmless from all suits, damages, or liability resulting from the Student's use of the stated equipment.
